Regenerative Laser Therapy (RLT) is the clinical application of High-Intensity Laser Therapy (HILT) — high peak-power Nd:YAG pulses directed toward tissue repair and regeneration. Because it’s based on the Nd:YAG wavelength (1,064 nm), it penetrates deeper into tissue than traditional low-level lasers. Delivered on the DEKA SmartPerio, it produces three distinct effects in biologic tissue — including photomechanical and photoacoustic effects that, per DEKA, no other commercially available laser is reported to reliably produce at controlled, non-toxic intensity.

How HILT works

Three effects in biologic tissue — one only the SmartPerio can produce.

Because HILT is based on the Nd:YAG wavelength (1,064 nm), it penetrates deeper into tissue than traditional low-level lasers. That energy produces three distinct effects.

Effect one

Photochemical

Common to all lasers. Absorption of laser energy by chromophores activates the oxidative chain, primarily in the mitochondria — the resulting increase in ATP is the foundation of the effect. Vasodilation, enhanced wound healing, and inhibited neuronal activity have also been reported. In essence, it wakes the cells up so they can do what they were designed to do.

Effect two

Photothermal

Only a handful of high-power Class IV lasers — and the DEKA SmartPerio HILT laser — can induce this effect. Raising soft-tissue temperature increases blood flow through vasodilation and raises enzyme activity and the catabolic rate, promoting a healing and remodeling response. Low-level “cold” lasers cannot produce it.

Effect three

Photomechanical & Photoacoustic

Only a high peak-power Nd:YAG like the SmartPerio produces it. Short pulses (100 µs) at kilowatt peak power deposit energy fast enough to create a transient thermoelastic expansion — a controlled photomechanical (photoacoustic) stimulus, without tissue disruption. Transmitted through the extracellular matrix via the integrin–cytoskeleton network, it activates mechanotransduction pathways that drive fibroblasts, osteoblasts, and chondrocytes toward proliferation and repair. It’s proprietary El.En. technology — DEKA’s parent company — built over nearly two decades of research. No other commercially available laser is reported to reliably produce this effect at controlled, non-toxic intensity.

HILT stands for High Intensity Laser Therapy — an advanced, non-invasive laser therapy that utilizes the power of the Nd:YAG laser wavelength to stimulate tissue repair, reduce inflammation, and relieve pain.

They describe the same energy delivery. HILT (High-Intensity Laser Therapy) names the modality — high peak-power pulsed Nd:YAG. RLT (Regenerative Laser Therapy) names the goal — applying that energy toward tissue repair and regeneration. On the DEKA SmartPerio they run on the same handpiece; the difference is treatment intent and protocol, not hardware.

HILT is based on the Nd:YAG wavelength (1,064 nm), which lets it penetrate deeper into tissue than traditional low-level lasers.

HILT produces a photochemical effect (common to all lasers), a photothermal effect (limited to a handful of high-power Class IV lasers, including the SmartPerio), and a photomechanical (photoacoustic) effect that only a high peak-power Nd:YAG like the SmartPerio can induce.
